This notice explains what information this website collects and how it is used. It applies to healingwithchaya.com only. If you become a client, you will receive a separate Notice of Privacy Practices describing how your health information is protected under HIPAA.
Information you choose to share
When you submit the appointment request form, we collect the details you enter, such as your name, email address, phone number, preferred contact method, insurance carrier, preferred language and session format, and the days and times you are available.
Please do not include clinical or personal health details in the form or in emails. We can discuss what brings you in during your consultation.
How this information is used
- To respond to your request and schedule a consultation or session
- To contact you using the method you prefer
- To check whether your insurance plan is accepted
Your information is never sold, rented or shared for marketing purposes.
Where your information goes
Form submissions are processed by our website host, Netlify, and delivered by email to the practice. Emails sent to hello@healingwithchaya.com are forwarded to the practice's email inbox. Submissions are deleted from the website host periodically once they are no longer needed for scheduling.
Cookies and analytics
This website does not use advertising cookies or tracking pixels. We may use privacy-friendly, cookie-free analytics to count page visits in aggregate. These statistics do not identify you.
Emergencies
This website, its forms and its email address are not monitored for emergencies. If you are in crisis, call or text 988, or call 911.
